TWYMEEG (Imeglimin) 500 mg tablets
Description
First-in-class oral antidiabetic of the glimin class (tetrahydrotriazine) for type 2 diabetes. Developed by Poxel (France) together with Sumitomo Pharma; the official branded drug TWYMEEG is approved and marketed only in Japan (since 2021).
💊 Composition:
▪︎ Imeglimin hydrochloride — 500 mg per tablet
🔬 Mechanism:
▪︎ Modulates mitochondrial Complex I and restores the cellular NAD+ pool
▪︎ Amplifies glucose-stimulated insulin secretion — it works only when glucose is elevated, so the intrinsic hypoglycemia risk is low
▪︎ Reduces hepatic glucose output and improves insulin sensitivity in muscle and liver
▪︎ The only oral antidiabetic whose mechanism directly targets mitochondrial dysfunction
🧬 Evidence:
▪︎ TIMES phase 3 program (Japan): HbA1c reduction of about 0.9% vs placebo at 24 weeks
▪︎ Glucose-lowering efficacy comparable to DPP-4 inhibitors, without weight gain
▪︎ No metformin-type mechanism of lactate accumulation; by physician decision it can be used with reduced kidney function where metformin is restricted
🕒 Dosing: 1000 mg (2 tablets) twice daily, morning and evening, exactly as prescribed by your physician
⚠️ Prescription drug (Rx): requires physician assessment, prescription and lab monitoring. Most common side effects are gastrointestinal (nausea, diarrhea), usually transient. Not recommended in pregnancy, breastfeeding and severe hepatic impairment.
