High prolactin: questionnaire

Your answers will show how soon to see a doctor, which common causes of high prolactin (a hormone made by the pituitary gland) need to be ruled out before an MRI, and which tests to get. The questionnaire is for women and men. It does not interpret your test result — the doctor will review it.
