🔬 How it works: unlike omeprazole and its relatives it is not a prodrug and needs no acidic environment to activate. It binds the gastric proton pump directly and reversibly, competing with potassium, so it works from the first dose, independently of meals, and keeps acid suppressed evenly — including overnight, when breakthrough acidity on PPIs is most common. Its effect also depends far less on the CYP2C19 genetic variant. 📊 Evidence: superior to lansoprazole for healing and maintaining remission in erosive oesophagitis (meta-analysis 2025); vonoprazan-based regimens outperformed standard PPI triple therapy for H. pylori in a US and European phase 3 trial (Gastroenterology 2022).
🕒 Regimens: 20 mg daily for erosive oesophagitis healing, 10 mg for maintenance; 14-day dual or triple therapy for H. pylori.
⚠️ Prescription-only. There is no reason to switch a PPI that already works; the niche is inadequate PPI response, severe erosive oesophagitis, night-time symptoms and H. pylori eradication. Long-term use requires justification.
