🔬 How it works: ordinary JAK inhibitors bind the ATP pocket, which looks similar across related kinases, so they inevitably touch neighbouring pathways. Deucravacitinib binds TYK2 regulatory (pseudokinase) domain instead — beside the lock, not inside it — holding the enzyme switched off. Regulatory domains differ far more between kinases, hence the selectivity. TYK2 carries the signals of interleukins 12 and 23, on which psoriatic inflammation depends. 📊 Evidence: POETYK PSO-1 and PSO-2 (JAAD 2023) — superior to both placebo and apremilast on PASI 75 and clear or almost clear skin; a five-year extension confirms durability.
