🔬 How it works: thyroid hormone receptors come in two types — beta dominates the liver, alpha the heart and bone. Resmetirom activates the beta type selectively and is taken up preferentially by the liver, so the liver cell burns stored fat without tachycardia or bone loss. 📊 Evidence: MAESTRO-NASH (966 patients, NEJM 2024) — a biopsy-controlled phase 3 trial in which the drug achieved both resolution of steatohepatitis and fibrosis improvement of at least one stage significantly more often than placebo. 🎯 Indication: MASH with fibrosis stages F2–F3 in adults, in addition to diet and exercise. NOT for cirrhosis (F4) and not for simple steatosis — the fibrosis stage must be established first.
