🔬 How it works: metamizole is not a classical NSAID. It has almost no anti-inflammatory action, and its analgesia is built differently — its active metabolites act largely within the central nervous system and it relaxes smooth muscle, which is why it works where an ordinary anti-inflammatory does not: colic, spasm, pain from a hollow organ. That is its place in the cabinet, not a higher rung on a strength ladder. 📊 Evidence: in the Cochrane single-dose review metamizole 500 mg gave an NNT of 2.4 (95% CI 1.8-3.1). That interval is the widest of the five and spans both the top and the bottom of the list — the evidence base here is the thinnest, not the strongest.

⚠️ Prescription-only, chosen by a physician. The known risk is agranulocytosis — very rare, no more than 1 in 10,000, but abrupt. The European Rote-Hand letter of 9 December 2024 abandoned routine blood monitoring precisely because the reaction develops faster than a scheduled test would catch it, and tightened the contraindications instead. Stop at once and have a full blood count taken for: fever, chills, sore throat, or painful sores in the mouth, nose, throat, genital or anal area. On antipyretic use and while on antibiotics these signs can be masked.
