🔬 How it works: it intercepts the aldosterone signal at the mineralocorticoid receptor in kidney tissue, where that signal drives inflammation and fibrosis. Unlike spironolactone it is non-steroidal, so it does not bind sex-hormone receptors and does not cause gynaecomastia or cycle disturbance. 📊 Evidence: FIDELIO-DKD (5,734 patients, NEJM 2020) — reduced risk of kidney disease progression; FIGARO-DKD (7,437 patients, NEJM 2021) — reduced cardiovascular events.
⚠️ Prescription-only. Requires baseline potassium, creatinine with eGFR and urine albumin-to-creatinine ratio; potassium is rechecked at 4 weeks and at every dose change. Not started with potassium above 5.0 mmol/L or eGFR below 25 mL/min. Added on top of an ACE inhibitor or ARB, not instead of it.
