🔬 How it works: since the 1950s every antipsychotic has blocked dopamine D2 receptors, which works but brings movement disorders, raised prolactin, weight gain and sedation. Cobenfy does not touch dopamine receptors. Xanomeline activates muscarinic M1 and M4 receptors, and activating M4 indirectly reduces excessive dopamine transmission where it is excessive. Xanomeline itself is thirty years old and was abandoned in the 1990s over body-wide cholinergic effects; trospium — which blocks the same receptors but barely crosses into the brain — was added purely to quench those peripheral effects. 📊 Evidence: EMERGENT-2 (Lancet 2024) in 252 participants — PANSS total fell 21.2 versus 11.6 points on placebo over 5 weeks (difference −9.6; p<0.0001; effect size 0.61), with extrapyramidal symptoms at 0% in both arms and weight gain at 0% versus 1%. A 52-week extension (Am J Psychiatry 2026) confirms durability.
