🔬 How it works: in this disease the heart is not weak but excessively strong — too many cross-bridges form between actin and myosin, so the muscle over-contracts and the thickened septum blocks outflow into the aorta. Mavacamten reduces the fraction of myosin ready to engage: contraction moderates, relaxation improves and the obstruction eases. 📊 Evidence: EXPLORER-HCM (Lancet 2020) — significant improvement in functional class and exercise capacity versus placebo.
⚠️ Prescription-only, and monitoring is what makes it safe: echocardiography before starting, during titration and every 12 weeks, because excessive suppression of contractility means heart failure. In the US it is dispensed under a restricted-access REMS programme. Contraindicated in pregnancy; significant CYP2C19 and CYP3A4 interactions.
