🔬 How it works: the two cyclooxygenases do different jobs. COX-1 works in the stomach lining and platelets all the time; COX-2 is switched on where inflammation begins. Non-selective drugs block both, which is why the stomach pays for the analgesia. Etoricoxib is the most selective of the licensed coxibs and leaves COX-1 largely alone — the trade-off is that the same selectivity shifts the balance between clotting and its restraint, which is where the cardiovascular caution comes from. 📊 Evidence: in the Cochrane single-dose review the NNT for etoricoxib 120 mg was 1.8 (95% CI 1.7-2.0) — the best figure of the five non-opioid analgesics in this catalogue.
